Appointment Policy

    1. Please provide payment in full at the end of the session.
    2. If your services are managed by a company that I have agreed to file
      for, please pay your co-pay or out-of-pocket portion at the beginning
      of the session and I will file for the remainder. Also, your signature
      will indicate that you:

      • Authorize use of personal identification information on all of your insurance submissions.
      • Authorize the release of information to your insurance company.
      • Understand that you are responsible for the full amount of
        your bill for services provided.
      • Authorize direct payment to me.
      • Permit a copy to be used in place of the original.
  1. If you should need to cancel or reschedule an appointment, please
    let me know as far in advance as possible. You will be charged the
    full fee for appointments missed or cancelled with less than 24 hours
     Medical insurance does not cover missed appointments.
  2. A $25.00 service fee will be charged on any return checks.
  3. I cannot guarantee availability in case of an emergency.
  4. Confidentiality: I will not share any information regarding your services
    without your prior knowledge and explicit consent, except as required by
    law or under the following circumstances: situations where I believe
    there is potential damage to self or others, suspected child or elder
    abuse or neglect, or if I am required to comply with a subpoena or
    court order.
